• We are located inside Performance Training Systems facility at 1515 W. Hubbard St.

• The entrance is located in the back of the building.

• From Hubbard St. you can get to the back of the building through the alley or you can access the parking lot off Kinzie St.

• We are located at 2934 Central St. Suite C2

• At the corner of Central and Hurd Streets.

• Our entrance is on Hurd St.

• Yes, the Chicago location has free street parking on Hubbard and Armour streets. Additionally there is a free parking lot (shared with Altitude Trampoline Park) located at 1514 W. Kinzie St.

• Evanston has free parking on Hurd St. just outside the clinic entrance.

Illinois is a direct access state for physical therapy, a referral is not required to initiate services.

• Yes, we are in network with Blue Cross Blue Shield-PPO.

• We are out-of-network for all other insurances.

• We have cash pay rates ($200 for a 1 hour initial evaluation and $160 per hour treatment session thereafter.

• We can provide a superbill for you to submit to insurance for potential reimbursement.
